It is especially useful in early-stage breast cancer (Luminal A and Luminal B, ER+, HER2\u2013, LN\u2013) patients to determine whether adjuvant chemotherapy is necessary after surgery.<\/p><p class=\"translation-block\">Eligible patient groups include Luminal A and Luminal B breast cancers that have the following characteristics:<\/p><ul><li><span style=\"font-weight: 400;\">Hormone receptor positive (ER+)<\/span><\/li><li><span style=\"font-weight: 400;\">HER2-negative<\/span><\/li><li class=\"translation-block\">No spread to the axillary lymph nodes (LN\u2013)<\/li><\/ul><p><span style=\"font-weight: 400;\">This test is done by analyzing a 21-gene assay on surgically removed tumor tissue to calculate a Recurrence Score (RS), which is used to indicate the risk of recurrence and the likelihood of benefiting from chemotherapy.<\/span><\/p><h2><span style=\"font-size: 24px;\"><b>Interpreting the Recurrence Score (RS)<\/b><\/span><\/h2><p class=\"translation-block\">Oncotype DX results are scored from 0\u2013100, with interpretations based on data from the TAILORx trial (NEJM 2018).<\/p><p><b>RS 0\u201310 Low risk <\/b><\/p><ul><li class=\"translation-block\">Endocrine therapy alone is sufficient; chemotherapy is unnecessary.<\/li><\/ul><p><b>RS 11\u201325 Intermediate risk <\/b><\/p><ul><li><span style=\"font-weight: 400;\">Women \u226550 years: Chemotherapy offers little to no benefit \u2192 Endocrine therapy alone.<\/span><\/li><li class=\"translation-block\">Women &lt;50 years: If RS is close to 21\u201325, chemotherapy may offer some benefit \u2192 requires individualized assessment.<\/li><\/ul><p><b>RS \u226526 High risk <\/b><\/p><ul><li class=\"translation-block\">Chemotherapy combined with endocrine therapy is recommended.<\/li><\/ul><h2><span style=\"font-size: 24px;\"><b>Supporting Evidence: TAILORx Trial<\/b><\/span><\/h2><p class=\"translation-block\">Population: ER+, HER2\u2013, LN\u2013 breast cancer patients.<\/p><p class=\"translation-block\">Intervention: Endocrine therapy alone vs. endocrine therapy + chemotherapy.<\/p><p><b>Results:<\/b><\/p><ul><li class=\"translation-block\">RS 11\u201325 (women \u226550 years): Chemotherapy did not improve survival outcomes.<\/li><li class=\"translation-block\">RS \u226526: Chemotherapy significantly reduced recurrence.<\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-03fc15b e-flex e-con-boxed e-con e-parent\" data-id=\"03fc15b\" data-element_type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-36c3cc6 elementor-widget elementor-widget-text-editor\" data-id=\"36c3cc6\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<h2><span style=\"font-size: 24px; color: #000000;\"><b>Limitations of Oncotype DX<\/b><\/span><\/h2><ul><li style=\"font-weight: 400;\" aria-level=\"1\" class=\"translation-block\">Applicable mainly to ER+, HER2\u2013, LN\u2013 cases.<\/li><li style=\"font-weight: 400;\" aria-level=\"1\" class=\"translation-block\">High cost (approximately 120,000\u2013150,000 THB in Thailand) and not covered by universal healthcare or social security schemes.<\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"color: #000000;\">Not suitable for Triple Negative or HER2+ breast cancer.<\/span><\/li><\/ul><h2><span style=\"font-size: 24px; color: #000000;\"><b>Who is suitable for Oncotype DX testing?<\/b><\/span><\/h2><ul><li style=\"font-weight: 400;\" aria-level=\"1\" class=\"translation-block\">Patients with early stage breast cancer (Stage I\u2013II)<\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400; color: #000000;\">The tumor is \u2264 5 centimeters in size.<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400; color: #000000;\">No or only minor lymph node metastasis.<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"color: #000000;\">The cancer is ER+\/PR+ and HER2\u2013.<\/span><\/li><\/ul><p class=\"translation-block\">Oncotype DX is a gene test that helps patients and their doctors tailor treatment options for early-stage breast cancer, reducing the risk of unnecessary chemotherapy and increasing confidence in treatment.<\/p><p class=\"translation-block\">Despite its high cost and lack of government coverage, Oncotype DX is an important tool. Especially in patients who do not want to receive chemotherapy, if you or someone close to you is in a high-risk group, you should consult an oncologist to determine the appropriateness of this
