Can Liver Cancer Be Treated Without Surgery?

Liver cancer (HCC: Hepatocellular Carcinoma) is one of the most common cancers in Thailand, especially among those with chronic liver disease or hepatitis B/C. A common question among patients and their families is: “Can liver cancer be treated without surgery?”

In this article, we’ll explore the modern non-surgical treatment options based on the international BCLC staging system and discuss survival outcomes at each stage.

Is Surgery Always Necessary for Liver Cancer?

Not always. Especially in early stages, non-surgical treatments can offer long-term control and even potential cure.

Treatment decisions depend on:

  • Tumor size and number
  • Liver function (Child-Pugh, ALBI score)
  • Overall health (Performance Status: PS)
  • Cancer spread (localized vs metastatic)

Non-Surgical Treatment Options by Stage

1. Very Early Stage (Stage 0)

  • Single tumor ≤2 cm
  • Preserved liver function and PS 0
  • Treatment: Ablation (RFA or MWA)
  • Expected survival: >5 years
  • Alternative: Liver transplantation (if eligible)

2. Early Stage (Stage A)

  • Up to 3 nodules, each ≤3 cm
  • Treatment: Ablation or Liver Transplantation
  • Expected survival: >5 years
  • Ablation is suitable for those who are ineligible for surgery or transplant

3. Intermediate Stage (Stage B)

  • Multiple tumors with good liver function
  • Treatment: TACE (Transarterial Chemoembolization)
  • Expected survival: >2.5 years
  • May be downstaged for transplant in some cases

4. Advanced Stage (Stage C)

  • Vascular invasion or metastasis
  • Treatment Optionsยาระบบ (Systemic Therapy)
    • Atezolizumab + Bevacizumab
    • Sorafenib, Lenvatinib, Durvalumab, Cabozantinib
  • Expected survival: >2 years
  • Second- and third-line options available if first-line fails

5. Terminal Stage (Stage D)

  • Advanced cancer with poor liver function
  • Treatment: Best Supportive Care (BSC)
  • Expected survival: ~3 months
  • Focus on palliative care and symptom relief
