Microsatellite Instability (MSI): What Is It?

Microsatellite Instability (MSI) is a genetic abnormality that occurs when the DNA repair system fails, leading to changes in microsatellites—repeated sequences of DNA. MSI is closely linked to the development of various cancers, particularly colorectal, endometrial, and gastric cancers.

How MSI Occurs

Microsatellites are short, repeated DNA sequences, such as AAAAA or CGCGCG. These regions are prone to errors during cell division. Normally, the body’s Mismatch Repair (MMR) system corrects these errors.

However, when the MMR system malfunctions, errors in microsatellite regions go unrepaired, resulting in MSI. Over time, this instability can cause cells to become cancerous.

Types of MSI

MSI can be classified into three types based on the level of instability:

  • MSI-High (MSI-H): Significant changes in microsatellites, often seen in certain types of cancers.
  • MSI-Low (MSI-L): Minor changes in microsatellites.
  • Microsatellite Stable (MSS): No changes in microsatellites.

MSI and Cancer Development

MSI is strongly associated with the development of cancers, especially in individuals with Lynch Syndrome, a hereditary condition that increases the risk of colorectal cancer. Additionally, MSI testing plays a crucial role in determining treatment plans, such as using Immunotherapy, which is particularly effective in patients with MSI-H tumors.

MSI testing is performed by analyzing tumor DNA to detect instability in microsatellite regions. This test helps doctors create more effective treatment plans and predict cancer prognosis.
